JOB SUMMARY:

An individual employed to identify, provide for, and accept responsibility for the fulfillment of resident's total nursing needs. This includes all nurse practice activities such as assessment, medication administration and monitoring, documentation, management of other nursing employees, etc.

This position is accountable to the Director of Nursing.

ESSENTIAL JOB FUNCTIONS:

1. Maintains and actively promotes effective communication with residents/patients/clients and family members/personal representatives.

2. Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.

3. Responsible for the assessment, preparation, coordination, and implementation of the total plan of care.

4. Provide regular supervision & direction of other nursing staff and their provision of care.

5. Administers and monitors the use of medications and maintains current knowledge base.

6. Observes, assesses and documents resident status utilizing recognized notification practices/policies, including 24 hour report and electronic medical record.

7. Notifies physician of change in condition and assists with physician visits.

8. Performs medical treatments and evaluates their effectiveness.

9. Will follow all department safety rules in performance of all duties.

Requirements:

QUALIFICATIONS

1. Commitment to quality programs and services for the residents/patients/clients.

2. Excellent communications and human relation skills.

3. Ability to relate well to the residents/patients/clients, family members/personal representatives.

4. Ability to maintain and protect the confidentiality of information.

5. Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.

6. Mathematical ability to determine medication and fluid calculations.

7. Ability to speak, hear, and write legibly to communicate with residents, families and staff.

8. Ability to organize tasks and function independently.

EXPERIENCE AND TRAINING

1. Current RN registration in the State of Wisconsin

2. Preferred past experience as a Certified Nursing Assistant
